The time has finally come to re-open!!!

 

We are planning to re-open on the 15th of June ...pending... we have PPE (personal protective equipment) in our office in stock. FEMA has had an severe effect on PPE availability and continues to be a problem.

 

As per guidelines from the recent CDC and ADA, the following symptoms list must be reviewed by you prior to coming to our office. We are following CDC and state regulations for pre visit screening as well as social distancing at our office. The list of symptoms are:

 

Fever or chills

Cough

Shortness of breath or difficulty breathing

Fatigue

Muscle or body aches

Headache

New loss of taste or smell

Sore throat

Congestion or runny nose

Nausea or vomiting

Diarrhea

 

**If any of these symptoms are present before your appointment please lets us know so we can reschedule (currently CDC recommends min 2 weeks).  If you have been diagnosed then a 1 month waiting period will be needed.**
